Outcome measurement tools
At
Grace
Living
Supported
Homes
we
are
always
looking
for
ways
to improve the quality of our care and rehabilitation services.
As
part
of
this
process,
we
will
introduce

Recovery
Star,
an
outcomes-based
measurement
tool
recognised
by
the
Department
of
Health
and
mandated
by
the
department’s
Commissioning

Quality

Innovation

framework.

tool

used
in
our
mental
health
services
and
we
will
also
try
to
extend
it
to
our
learning
disability
services
as soon as it is practicable to do so.
Using Recovery Star will enable us to support service users –

them

recovery

of
their
mental
health,
living

and

Supporting Living
Supported living is the arrangement whereby someone who
already has (or wants to have) their own tenancy or own home,
also has support from a “Care and Support” provider to help them
live as independently and safely as possible. They will receive
support to live their everyday lives which might include help with:

Managing bills and money

Cooking and healthy eating
•
Shopping
•
Getting a job
•
Learning new skills
•
Personal care and wellbeing

Managing medication

Accessing sports, leisure and social activities
People who live in supported living can live in a variety of different
settings, for example:

With another person or others they have chosen to live
with in shared accommodation eg a shared house
where they all contribute to the bills and upkeep of the
communal areas

On their own in a flat/house or bungalow, etc.
In supported living tenants are responsible for providing any
furniture, both for their own room and for any communal or
shared areas. Tenants are also responsible for paying their own
bills including utility bills, buying their own food, drink and clothes
and covering the cost of any activities they want to do. At Grace
Living Supported Homes a key part of the care and support
service we provide is about supporting tenants to understand
their responsibilities and support them to effectively and safely
manage these areas.

Domiciliary Care
Care In Your Home – Your Choice:
We offer a unique service of personalised care visits from check-in
visits to full live-in support.
We, at Grace Living Supported Homes, specialise in the provision
of high-quality homecare and support to keep you safe and
comfortable in your own home. We cater for all age groups
(younger adults to elderly individuals) with a multitude of needs
ranging from physical support to Mental Health and Learning
Disabilities.
We want to listen to your wishes, preferences and desires about
the support you require and how you want it to be given to you.
Our staff members want to support you to maintain your chosen
lifestyle. We believe that each customer is unique, with their own
individual preferences and needs. So, we keep you in control and
provide you with the homecare and support that you want, where

We offer flexible care services for you to choose from. These
services could be anything from a 30-minute visit to live-in-care
and ongoing 24-hour staff presence support with a core staff team

Our Services
Grace Living Supported Homes also has the following in place
to ensure we are to continually improve the quality of our

These are reviewed regularly at local management level:

A comprehensive training programme monitored by the
organisation and the individual staff member to agreed
targets. This includes mandatory training as well as
training specific to the needs of each service and service
users.

Foundation and advanced management development
programs to enable staff progression and provide
experienced and trusted Grace Living Supported Homes
staff with opportunities to manage new services

Registered Manager will carry out annual inspections to
the CQC format in addition to regular monthly
monitoring visits. A planned schedule of unannounced
out of hours visits is also undertaken by the Registered
Manager and other members of the Senior
Management Team.
•
Service users are employed as “Expert Auditors” to help
the Manager during monthly monitoring visits. They
produce a report which is circulated to the Manager.
•
Each member of staff carries a card with the Managers
contact details ensuring the ability to whistle blow at
any time. The website includes a facility to whistle blow
easily through the home page and directly to the
Registered Manager.
•
Grace Living Supported Homes has a strong focus on
recruitment to ensure that staff vacancies are always
low. This helps to ensure we always know our staff and
that they are appropriately trained.

purchasers and be transparent with respect to fee
breakdowns so that the purchasing body is very clear
about the levels of support and service they are

All staff have DBS (Disclosure Barring Service) checks
and references which meet Grace Living Supported

Robust systems are in place to ensure that new staff
cannot put our service users at risk.

All service users have activity timetables to ensure that
they are fully engaged and occupied; this is overseen by
the Registered Manager.

Inspections
All of Grace Living Supported Homes services which require CQC
registration will be registered and inspected by the Care Quality
Commission (CQC).

Internal Inspections:
At Grace Living Supported Homes we believe that the people using
our services are best placed to identify what we are doing well in
providing quality outcomes for service users and what we could
do better.
So, in addition to the Internal Inspections carried out by our
Quality Team, we also operate an Expert Auditors scheme. These
individuals who use our services are employed to carry out one
task of management monitoring per month, and report back with
their findings.
The feedback of some of our Expert Auditors will be presented to
Grace Living Supported Homes Senior Management and the
feedback will be posted on this page.
We will value the insight the Expert Auditors will provide into the
way the services work. As an organisation, we will always look to

Support to access educational and
employment opportunities
We will strive to ensure that all our service users will have
individually-designed packages of education, employment and
leisure opportunities, devised to meet their specific needs and
abilities as well as their likes and dislikes.
The aim is to build on existing skills and support each individual in
their continued personal development.
Wherever possible, programmes will be devised in association
with the service user, as well as relevant professionals such as
speech therapists local colleges and music, drama and art
therapists. Service users can also take advantage of a wide range
of community resources, including colleges, sports and
leisure facilities, pubs, clubs and restaurants, supported
workshops and vocational opportunities

Residential
A residential placement typically consists of a property, located
within an ordinary community setting (owned by the care
provider) that is shared between a group of individuals with
similar needs. Care is usually provided on a 24 hour basis, with
either waking or sleep in night support, depending on the needs
of the individuals. There are also staff on shift throughout the day
to support individuals in all aspects of their life.
In residential placements, the package may include positve
behaviour support from in-house teams and the provision of a full
programme of activities. Usually, all food and household bills are
paid for, all standard furniture provided and
maintenance of the property, including safety adaptations, repairs
and the installation of assistive technology is all the responsibility
of the Care Provider.

Supported Living Placements
‘Supported Living’ is a term used whereby an individual has their
own apartment or house (tenancy) which is usually rented
(although we do have one example of where the family have
purchased a [property on behalf of their children). People may
choose to live on their own or to ‘house share’ with peers.
Alongside sourcing a property, (which can be through local council
housing stock, a housing association or a private landlord), the
individual will have their care support needs assessed by the
funding authority.
Providers will then be invited to meet the individual and their
representatives who will select which provider they want to deliver
this care.
In terms of how Supported Living placements are funded, this
works as follows:
The direct care (as assessed by the funding authority, most likely by
your allocated social work or care coordinator) is paid by the local
council or NHS.
The property rental costs are covered by housing benefit.
Food, personal effects, furniture and utility bills are paid for by the
individual through their allocation of benefits and any personal
monies they may have. Obviously, if the person is able to / wants to
live with peers, some costs can be shared.

Community Support
‘Where the individual is still living within the family home and the
family perhaps need the individual cared for whilst they are at
work / attending to other family members or they cannot cope
with caring for the individual by themselves, the care of package
the home, the individual can be allocated support to access
community based activities and opportunities.
For both ‘Supported Living’ and ‘Community Support’ packages of
care, the number of hours allocated to the person is totally
individual and based upon need. For some, the care package may
consist of as little as one or two hours a week. For others however,
the support delivered may be on a
24-hour basis, with either ‘waking’ or ‘sleep-in’ night support.
Deciding on which package of support is right – be that a
residential placement, supported living or community support is
totally dependent on what works best for individual, at that
particular time in their life. Some may find that the residential
option works best for them on an on-going basis. Others may
move on to a supported living placement following a period in a
residential home. Others might move directly from home or school
in to their own supported living tenancy. As with any of us,
different styles of living suit different people!

How do I find a good Provider?
‘There are various website that will direct you to care providers.
One of the most popular is www.carehome.co.uk. Also, your local
council may have advice on their own website. Alternative, you
can ‘Google’ the key phrase or words that apply to type of care and
support you are looking for.
Once you have found a provider you are interested in, look them
up on the Care Quality Commission (CQC website). CQC is an
independent body who monitor, inspect and rate care providers.
Always visit the care provider. Find out about their values, meet
the manager and staff and really get a ‘feel’ for the
organisation. Never be afraid of asking questions – as many as you
like and as frequently as you need.
Before offering to provide a placement or care package, the
provider should always complete a thorough assessment so as to
ensure that they fully understand the person being referred and
are confident that they can meet their needs.

of the people we support and for those individuals, who are not
able to advocate for themselves, we welcome the advice of friends
and family so that we can support the individual to lead a lifestyle
that encompasses their likes, dislikes and all that is important to
them.

At Grace Living Supported Homes, we maintain a high staff to
service user ratio and our team of enthusiastic, skilled and
committed employees provides 24-hour support to all our service
users. All homes have a management team of a registered
manager and a deputy and/or assistant manager who supervise
and support their team.
Our staff help us to provide a supportive and stimulating
environment that is designed to meet specific needs. They support
service users in occupational, educational and recreational
activities – making use of both home and
community-based facilities – with a focus on activities that relate to
an individual’s interests and enhance their communication and

Employees are offered time off on full pay to pursue relevant

choice of nutritious and wholesome meals, catering for
special diets, any cultural or religious needs and individual
preferences. When possible, service users are supported and
encouraged to produce their own meals. Some individuals,
however, require increased assistance and supervision from
staff.
